Join us as we reflect on the significant accomplishments of the 12th Statement of Work for the Quality Innovation Network-Quality Improvement Organization (QIN-QIO) program with CMS. From improving nursing home care and reducing opioid-related adverse events to increasing COVID-19 vaccination rates, we've made remarkable strides together. As we look forward to hearing more about the 13th Statement of Work in 2025, we extend our heartfelt gratitude to our dedicated partners for their unwavering support and commitment to enhancing beneficiary outcomes. QIN-QIO PROJECT SUCCESSES
Discover our national and local successes, where collaborative efforts have led to impactful results in healthcare quality and safety for beneficiaries.
National Successes
- The QIN-QIOs worked with more than 11,600 nursing homes.
- Rapid response COVID-19 reduction efforts led to 6,700 nursing home residents’ lives saved in the first year of the pandemic and continues to reduce COVID-19 infections among nursing home residents. Nursing homes who worked with QIN-QIOs had:
- 25.2% fewer resident COVID-19 infections
- 26.4% fewer resident hospitalizations
- 24.3% fewer resident deaths
- The QIN-QIOs had more than 40,000 one-on-one encounters as part of a targeted COVID-19 vaccine response.
- Nursing home resident deaths from COVID-19 fell by 55%.
- Working with community partners, the emphasis on preventive health led to a 12.5% reduction in hospitalizations and 20% reduction in all-cause readmissions.
Pennsylvania and West Virginia Nursing Home Care
Together, we have achieved significant relevant improvements over baseline data in nursing home care during the past five years, including:
- A 10.5% improvement in reducing opioid-related adverse events
- A 31.1% decrease in hospitalizations due to C. Difficile Infection
- Notable increases in COVID-19 vaccination rates among residents and staff, at 60.0% and 36.0% relative improvements over baseline, respectively.
Pennsylvania and West Virginia Partnership for Community Health Coalition Successes
Together, through our Partnership for Community Health Coalition initiatives, we have achieved significant relevant improvements over baseline data during the past five years, including:
- Reducing opioid-related adverse drug events and deaths among high-risk patients by 10.5%
- Improving blood pressure control among Medicare beneficiaries with hypertension by 34.6%
- Reducing the percentage of Medicare beneficiaries with diabetes who had hemoglobin A1c levels above 9.0% by 88.2%
- Lowering hospital utilization among Medicare beneficiaries by 11.6 per 1,000 beneficiary years
- Decreasing the total number of 30-day hospital readmissions for Medicare beneficiaries by 14.6%
