For over 50 years, we’ve partnered with healthcare providers, public agencies, and community organizations to improve health outcomes across the U.S. With nearly 500 community-based partnerships, we've positively impacted care for 18.2 million Veterans and 3.3 million Medicare beneficiaries through national and local initiatives.
Our quality improvement efforts help prevent avoidable events and reduce system inefficiencies, leading to real savings. From 2020 to 2024, our work helped avoid 112,664 hospitalizations and 23,933 readmissions, contributing to $1.7 billion and $423 million in savings, respectively.
We combine data insights with continuing education to improve care delivery. Since 2020, we’ve driven quality improvement by analyzing and reporting on 1.5 million episodes of care across our customer populations, delivered 6,500+ hours of education, and supported 47,400+ dialysis patients annually.
