Henrico, V.A. | APRIL 11, 2023

Quality Insights was recently featured nationally in the Patient-Centered Outcomes Research Institute’s (PCORI) Engagement Award’s Monthly Digest for the success of the project Use of a Virtual Education Platform to Disseminate PCORI-Funded Evidence into Dialysis Practice Settings.”

This project features Quality Insights’ 5-Diamond Patient Safety Program, an automated, online, national educational resource.

Through the 5-Diamond Program, facilities can log in to an online portal (www.qualityinsights.org/5diamond) to sign up for, view and download patient safety modules to provide free education within their facilities. They can track progress and receive certificates upon completion of the program.

“Dialysis facility staff must balance providing care for individuals with complex conditions while simultaneously striving to meet metrics reflective of high quality care,” Brandy Vinson, executive director of Quality Insights Renal Network 5 said. “The 5-Diamond Patient Safety Program provides access to educational modules based off of current literature for the entire dialysis team, for free!”

As of December 2022, 2,541 (32%) dialysis facilities completed the care coordination/care transitions module and 319 (4%) completed the mental/behavioral health module. 192 (2%) dialysis facilities completed both modules.

“In an ideal world all dialysis facilities across the United States would enroll in the 5-Diamond Patient Safety Program and complete the four PCORI-funded educational modules,” Vinson said. “Evidence disseminated into practice in this way would positively impact dialysis staff to engage patients in their care to improve care coordination, identify and treat depression and prioritize advance care planning for all patients.”

Since 2014 the 5-Diamond Patient Safety Program has served an average of 2,675 dialysis providers annually.
