Diabetes Management Support
for West Virginians
Get Free Support for Your Health
Did you know that having diabetes or even prediabetes puts you at risk for chronic kidney disease (CKD)? You don’t have to wait for a diagnosis of kidney disease to get support. The West Virginia Chronic Kidney Disease (CKD) Prevention Program is here to help you manage diabetes, prevent complications, and protect your long-term health—starting today.
You’re Eligible If You Have:
- Type 1 or Type 2 Diabetes
- Prediabetes
- High blood pressure
- A family history of kidney disease
- Other risk factors for CKD
This means you don’t need to have CKD to qualify for free support through our program. If you're living with diabetes or prediabetes in West Virginia, especially in Boone, Greenbrier, Jackson, Mingo, Roane, or Wyoming counties, you can enroll today and take control of your health.
Most people are completely covered, but coverage can depend on your insurance. We’re happy to help you figure it out—just leave us a message in the sign-up form below!
