ABOUT THE PROGRAM
West Virginia Chronic Kidney Disease Prevention Program
The West Virginia Chronic Kidney Disease (CKD) Prevention Program aims to prevent and manage CKD by integrating registered dietitians into the care plan for West Virginians residing in the counties of Boone, Greenbrier, Jackson, Mingo, Roane and Wyoming. While the emphasis of this program is on those counties, all West Virginia residents are encouraged to participate.

What to Expect
Our six month program includes a short pre-test, the completion of weekly check-ins reports, and a final post-test to measure your progress. Most materials are sent by email, but printed versions are available if that’s easier for you—just ask the dietitian we pair you with!
1. Sign Up 💻
Sign up on the form found on this page. Once we receive your form, our program manager Jillian Pate will reach out.2. Meet Your Dietitian 👩💻
We'll match you with one of our incredible dietitians to help you set and manage your goals. You’ll also receive a welcome packet with everything you need to start building healthy nutrition habits right away.3. Complete Your Pre-Test 📝
Improving your health through nutrition starts with understanding your current status. Your pre-test results provide a snapshot of your health and help us create personalized goals just for you.4. Attend Virtual Weekly Check-ins👥
Meet with your renal dietitian virtually each week to talk through challenges, celebrate successes, and stay on track with your health goals.
5. Complete Your Post-Test📝
Take the post-test to see how far you’ve come! This final check-in helps measure your progress and highlights the improvements you’ve made in your health and habits.6. Celebrate Your Success 🎉
