What is data quality improvement?
CMS mandates dialysis facilities submit ESRD program administrative and clinical data into EQRS, in order for dialysis facilities to meet portions of the Conditions for Coverage for ESRD Dialysis Facilities. All Medicare-certified dialysis facilities, as well as Renal transplant and Veterans Administration facilities, can submit ESRD program administrative and clinical data to CMS in EQRS. EQRS functions as an important data source for ESRD QIP performance measures.

Data Stars must meet the following timeliness measures:

90% on-time submissions of the 2 data quality goals

Meeting one or more of the data quality improvement goals (listed below)

CMS-2728 forms for newly diagnosed chronic patients submitted into EQRS within 45 days of the patient’s admission to your facility
-
CMS-2746 forms submitted into EQRS within 14 days of the patient’s date of death for 3 consecutive months
