Patient Engagement Opportunities

Overview

Quality Insights Renal Network 5 believes it is important that patients be involved in their care, as well as in the work we do. Patients and families can actively participate in Network activities in different ways:

Patient Liaison- Acts as a bridge between the Network and other patients in the facility. Patient liaisons can also work with the Network to assist the facility in quality assurance and performance improvement (QAPI) and peer mentoring.

Patient & Family Advisory Council (PFAC)- Serves as the voice of dialysis patients and family members, expressing concerns, needs and values of patients. Members often act as subject matter experts (SME) on workgroups that are formed to help in quality improvement projects.

Each of these groups drive change in the dialysis facilities with patients and families providing direction and actively helping the Network to improve the quality of care that all patients receive.  For more information on each opportunity, click the appropriate link below:

Please read the Patient Services brochure (PDF) to learn more about patient opportunities with Quality Insights.
