Quality Improvement & Quality Assessment and Performance Improvement (QAPI)

What is quality improvement?

Quality Improvement is a method of planning and implementing continuous improvements in systems or processes in order to provide quality health care reflected by improved patient outcomes.

Quality Insights bases its improvement process on the Model for Improvement developed by Associates in Process Improvement, and utilized by the Institute for Healthcare Improvement (IHI).

This model has two parts:

  • Establishing what is to be studied and how to measure the improvements; and

  • Using the PDSA cycle to test changes

Quality Assessment & Performance Improvement (QAPI)

Per § 494.110: Quality Assessment and Performance Improvement, of the Conditions for Coverage for ESRD Facilities, "The dialysis facility must develop, implement, maintain, and evaluate an effective, data-driven, quality assessment and performance improvement program with participation by the professional members of the interdisciplinary team."

In addition to the interdisciplinary team of professionals required by the Conditions of Coverage, facilities are strongly encouraged to include patients on the QAPI team.
