Million Hearts® Hypertension Control Champions
The Million Hearts® Hypertension Control Champions are clinicians, practices, and health systems that have successfully completed the Million Hearts® Hypertension Control Challenge. The Challenge is an opportunity for clinicians, practices, and health systems to demonstrate excellence in hypertension control. Hypertension Control Champions must reach 80 percent control rates among their hypertensive patients.
All of the hypertension champions made blood pressure control a priority. They achieved improved performance by implementing health information technology, home blood pressure monitoring, and medication adherence.
As of 2020, CDC has recognized 133 Hypertension Control Champions across 40 states and the District of Columbia.
Two PA Practices Recognized as Champions
In 2019, Million Hearts® selected 17 Hypertension Control Champions from across the country. Two of these champions hail from Pennsylvania - Burstein Medical Associates and Stolz and Hahn Family Medicine. Both practices are active participants in Quality Insights' Hypertension and Diabetes Control and Prevention Project.

Million Hearts® 2021 Hypertension Control Exemplars
Due to the COVID-19 pandemic there was no Hypertension Control Challenge in 2021, however, Million Hearts® did recognize health care practices and organizations across the U.S. as 2021 Hypertension Control Exemplars for implementing successful innovations during the pandemic.
Quality Insights was recognized as a 2021 Million Hearts Hypertension Control Exemplar for the work our team conducted with medical practices to ensure that hypertension control remained a priority through this difficult time.
