The Paul Coverdell National Acute Stroke Program
Participation
To participate in the Quality Insights and PA DOH Paul Coverdell National Acute Stroke Program, your practice must agree to do the following:

Designate an individual to act as the primary contact for Quality Insights.

Sign a Memorandum of Understanding (MOU) and Business Associate Addendum (BAA) that outlines the role of the practice and Quality Insights.

Treat patients at-risk for stroke with hypertension, hypercholesterolemia or previous stroke.

Submit annual quality reports for CMS #165- Controlling High Blood Pressure and CMS #22- Screening for High Blood Pressure and Follow-Up Documented

Participate in a workflow assessment and analysis regarding current hypertension and cholesterol processes within the practice.

Receive quality improvement recommendations and technical assistance from your Quality Insights Practice Transformation Specialist.

Consider best practice workflow modifications and Health Information Technology (HIT) improvements for implementation within your organization.

Implement evidence-based, team-based care (TeamSTEPPS)

Integrate AHA Get With The Guidelines®–Stroke framework
-
Facilitate coordination between community and clinical services
-

Share the educational materials provided with your staff and, as appropriate, patients.
